Acute epiglottitis complicating an emphysematous abscess.

چکیده
A 32-year-old man presented to the emergency department with a 2-day history of nonproductive cough, fever, severe sore throat, odynophagia, and dysphagia. A chest radiograph showed no significant finding, but the lateral view of the neck demonstrated ‘‘vallecula’’ and ‘‘thumbprint’’ signs (Figure 1). Significant laboratory findings were a white blood cell count of 22,000/mL with 93% neurtrophils and an elevated C-reactive protein at 51.6 mg/L (49.1 nmol/L). Direct laryngoscopy revealed a swollen epiglottis (Figure 2). The neck computed tomographic (CT) scan demonstrated a swollen epiglottis with air bubbles in the heterogeneous lesion consistent with acute epiglottitis complicated by an emphysematous abscess (Figure 3). Acute epiglottitis is a bacterial or viral infection of the supraglottic structures that can lead to upper airway obstruction, although this is rare in adult cases. Clinical manifestations include fever, sore throat, muffled voice, dysphagia, odynophagia, drooling, dyspnea, and stridor. Emphysematous abscess is rare, with an incidence of 0 to 3%. Vallecula and thumbprint signs on the lateral view of the neck should aid in diagnosing acute epiglottitis. CT can assess the extent of the disease and identify possible complications. Direct laryngoscopy will show the adjacent structures of epiglottis. The most common pathogen is Haemophilus influenzae type B in children and Streptococcus pneumoniae in adults, but positive rates of throat swab and blood cultures are uncommon. Although radiologic studies demonstrated an emphysematous abscess in this patient, medical treatment was initiated without intubation because there were no signs of respiratory distress.

Airway Obstruction Secondary to Emphysematous Epiglottitis: A Case Report
BACKGROUND Despite a shift in the epidemiology of epiglottitis following the introduction of the Haemophilus influenzae type B vaccine, acute infectious epiglottitis continues to be a relatively common and potentially life-threatening infection in adults. Epiglottitis complicated by abscess is rare, and emphysematous abscess is even less common. Early diagnosis and intervention is paramount in ...
متن کاملParaglottic and Pre-epiglottic Space Abscess in Adults: Report of Two Cases
Introduction: Acute epiglottitis or supraglottitis is a rapidly progressing upper respiratory tract infection that can often threaten the airway patency. Epiglottic abscess that expands to the paraglottic (PGS) or preepiglottic (PES) space and acute airway obstruction constitute rare complications, exclusively presented in adults. Case Report: We report two cases. In t...
متن کاملSudden Death from Acute Epiglottitis and Epiglottic Abscess in Adult
Acute epiglottitis and epiglottic abscess are forms of acute inflammation of the epiglottis and adjacent structures. They are potentially life-threatening conditions and likely to be underdiagnosed or misdiagnosed in adults. Traditionally, acute epiglottitis is regarded as a disease of children; however, its prevalence among children has decreased owing to vaccination. In contrast, the prevalen...
متن کاملAn Unusual Case of Acute Epiglottic Abscess
A 48 year old man presented to our outpatient department with the history of absolute dysphagia, and fever for two days. On examination patient was febrile with drooling of saliva. Oropharynx was minimally congested. Indirect laryngoscopy revealed odematous epiglottis with pus pointing over the lingual surface. Radiography of soft tissue, neck lateral view, showed the thumb sign which suggeste...
متن کاملA reappraisal of the radiologic findings of acute inflammation of the epiglottis and supraglottic structures in adults.
PURPOSE To evaluate the radiologic characteristic of acute inflammation of the epiglottis and supraglottic structures in adults. METHODS The clinical and radiographic findings in 27 adult patients with epiglottitis (average age, 43 years; range, 28 to 81 years) were compared with those of a control group of asymptomatic subjects (n = 15; average age, 48 years; range, 24 to 79 years). Unusual ...
